DEFINITION

FITNESS PLUS is a fitness center, with Mega Fitness facilities, and one of the largest in Bali. FITNESS PLUS MANAGEMENT is all employees who are part of the company, each of whom is responsible for organizing and operating all the equipment and facilities owned by FITNESS PLUS. FITNESS PLUS MEMBERS are persons who have registered as MEMBERS, and have fulfilled various obligations as MEMBERS. FITNESS PLUS guests are everyone who comes to exercise, excluding employees, MEMBERS and FITNESS PLUS. FITNESS PLUS operating hours are in accordance with the information listed and announced in the FITNESS PLUS area.

COMMON

1. FITNESS PLUS MEMBERSHIP can be transferred or forwarded or sold to other parties with a note to both parties directly come to the original branch and re-register with a penalty fee of IDR 250,000,-.
2. For those who want to become a FITNESS PLUS MEMBER, they are required to fill out the registration form and pay MEMBERSHIP and administration fees, as well as other FITNESS PLUS fees that have been determined by FITNESS PLUS management.
3. FITNESS PLUS MANAGEMENT reserves the right to refuse any request for FITNESS PLUS MEMBERSHIP, without giving any reason to the prospective FITNESS PLUS MEMBER.
4. FITNESS PLUS MEMBERS who wish to enter FITNESS PLUS without using a valid MEMBERSHIP Card, can only enter with the approval of FITNESS PLUS MANAGEMENT.
5. MEMBER and FITNESS PLUS guests are adults with a minimum age limit of 16 years. If the MEMBER and FITNESS PLUS guests do not meet the above criteria, then there must be a special agreement between the parents and the FITNESS PLUS MANAGEMENT, whereby the parents must be present and sign a MEMBERSHIP contract or permission for the guest.

FITNESS PLUS MEMBERSHIP

1. The period and type of FITNESS PLUS MEMBERSHIP is in accordance with what is stated in the MEMBERSHIP CONTRACT.
2. FITNESS PLUS MEMBERSHIP expires after the MEMBERSHIP period ends as stated in the MEMBERSHIP Contract or system revision if there is a time change due to leave, free MEMBERSHIP bonus, etc. If there is a cancellation before the MEMBERSHIP expires period, then the provisions of the FITNESS PLUS clause will be subject to penalties, based on the terms of termination of the MEMBERSHIP.

FITNESS PLUS MEMBERSHIP DUES AND/OR FEES

1. FITNESS PLUS MEMBERS are required to pay dues and/or fees charged when registering and such fees are non-refundable. FITNESS PLUS MANAGEMENT reserves the right to make adjustments to the applicable fees in FITNESS PLUS from time to time and will be announced to FITNESS PLUS MEMBERS through the media registered in the FITNESS PLUS area.
2. All fees paid by a FITNESS PLUS MEMBER, while he/she is a FITNESS PLUS MEMBER, cannot be withdrawn under any circumstances or conditions.
3. MEMBERSHIP starting in the middle of the current month will use the pro rate as the number of days outside the first month of membership.
4. All fitness plus membership fees cannot be reduced, even if the MEMBER does not use or visit FITNESS PLUS for any reason, including suspension of FITNESS PLUS MEMBERSHIP.
5. If any credit card authorization fails, FITNESS PLUS MANAGEMENT will contact the MEMBER to make direct payment of the MEMBERSHIP fee at FITNESS PLUS.
6. Any FITNESS PLUS MEMBER whose membership has been cancelled, may rejoin FITNESS PLUS by paying a fee, which has been applied by FITNESS PLUS MANAGEMENT. When MEMBERS want to become FITNESS PLUS MEMBERS again.
7. FITNESS PLUS MANAGEMENT reserves the right, from time to time, to make adjustments to the MEMBERSHIP AND other fees payable for each FITNESS PLUS MEMBERSHIP.

FREEZE FITNESS PLUS MEMBERSHIP

1. FREEZE FITNESS PLUS MEMBERSHIP can only be done after the first three months of the FITNESS PLUS MEMBERSHIP period.
2. FREEZE FITNESS PLUS MEMBERSHIP can only be given to 12-month MEMBERSHIP.
3. FITNESS PLUS MEMBERS can freeze MEMBERSHIP in FITNESS PLUS by paying the MEMBERSHIP FREEZE Fee, which is IDR 100,000 (one hundred thousand rupiah) every month, with a payment method that has been determined by FITNESS PLUS MANAGEMENT, or direct payment at FITNESS PLUS, especially for the pre-paid system.
4. FITNESS PLUS MEMBERS shall inform FITNESS PLUS MANAGEMENT, not less than 10 days before membership is frozen, with a minimum freeze period one month and a maximum of 12 months in a one-year MEMBERSHIP.
5. FITNESS PLUS members who freeze their membership cannot use the FITNESS PLUS facility.

TERMINATION OF FITNESS PLUS MEMBERSHIP

1. FITNESS PLUS MEMBERS are not allowed to terminate monthly MEMBERSHIP within the first 12 months.
2. FITNESS PLUS MEMBERS who decide to terminate their membership, must notify FITNESS PLUS MANAGEMENT and fill out a termination request form, not less than 10 days prior to the monthly FITNESS PLUS MEMBERSHIP fee bill.
3. FITNESS PLUS members who decide to terminate their membership while the membership freeze still exists, shall still pay the designated fee.
4. If FITNESS PLUS MEMBER terminates MEMBERSHIP before the first one-year period, then FITNESS PLUS MANAGEMENT has the right to provide administrative sanctions of IDR 550,000 (five hundred and fifty thousand rupiah) (including tax), as proof of termination before the one-year period ending with direct debit from the credit card, the same as the credit card used in the FITNESS PLUS MEMBERSHIP form, or in any other way decided by FITNESS PLUS MANAGEMENT.
5. For 1-month, 3-month and 12-month (annual) MEMBERSHIP, all fees paid are non-refundable.
6. FITNESS PLUS MANAGEMENT reserves the right to terminate FITNESS PLUS MEMBERSHIP by reason of any violation, crime or conduct that FITNESS PLUS MANAGEMENT finds contrary to moral decency and laws that may harm, whether fitness plus members, parties other than MEMBERS, or FITNESS PLUS MANAGEMENT itself.
7. With the expiration of FITNESS PLUS MEMBERSHIP, it does not mean that MEMBERS can be free from unfulfilled obligations for the management of FITNESS PLUS (if any).

ETHICS: DRESS, MORALS & HYGIENE

1. FITNESS PLUS MANAGEMENT has the property of a personal training program provided on FITNESS PLUS. Under no circumstances or circumstances shall FITNESS PLUS MEMBERS and/or FITNESS PLUS GUESTS be permitted to provide any training to other FITNESS PLUS MEMBERS and/or FITNESS PLUS GUESTS.
2. If the FITNESS Plus MEMBER and/or GUEST is found to have taken the action as indicated in point 1 above without the permission of FITNESS PLUS MANAGEMENT then FITNESS PLUS MANAGEMENT reserves the right to terminate FITNESS PLUS MEMBERSHIP and all fees paid by the MEMBER are non-refundable.
3. Sports bags are not allowed to be placed in the training room. If FITNESS PLUS MEMBERS and FITNESS PLUS GUESTS bring sports equipment per¬, then they must store them in the lockers provided.
4. During sports activities in the training area, FITNESS PLUS MEMBERS and/or FITNESS PLUS GUESTS must bring personal equipment needed by the FITNESS PLUS MEMBER and/or GUEST themselves, and provide towels. Upon completion of using the equipment, FITNESS Plus MEMBERS and/or GUESTS are required to maintain the cleanliness of the exercise equipment used by the FITNESS PLUS MEMBERS and/or GUESTS, especially after using the exercise equipment.
5. FITNESS PLUS MEMBERS and/or GUESTS are strictly prohibited from smoking in all FITNESS PLUS areas and bringing food and drinks in open packaging in the training area.
6. All damage to FITNESS PLUS facilities and equipment caused by FITNESS PLUS MEMBER and/or GUEST will be responsible and charged to MEMBER and/or related parties causing the damage.
7. Fellow FITNESS PLUS MEMBERS and/or GUESTS must respect each other and must not behave in accordance with FITNESS PLUS MANAGEMENT that may interfere with other FITNESS PLUS MEMBERS and/or OTHER FITNESS PLUS GUESTS.
8. All FITNESS PLUS MEMBERS and/or GUESTS must wear modest training attire and wear recommended sports shoes during workouts in the FITNESS PLUS training area.
9. FITNESS PLUS MEMBERS and/or GUESTS are not permitted to record and take pictures of activities on FITNESS PLUS, whether by using a mobile phone with camera features or any other type of recorder without permission from FITNESS PLUS MANAGEMENT.

LOCKER LOCKS AND TOWELS

1. FITNESS PLUS MEMBERS and/or GUESTS who use FITNESS PLUS facilities must return the locker lock and towel to the towel service desk upon completion of using it.
2. Lockers are only available for daily use, MEMBERS and/or FITNESS PLUS GUESTS are not permitted to leave valuables and in the event of loss will be personal responsibility.
   1. FITNESS PLUS MEMBERS AND/or GUESTS are charged a replacement fee for damage and/or loss according to the price applied.

GENERAL RULES

FITNESS PLUS guests include:

1. People who practice using various types of Vouchers in the form of 1-day training vouchers, trial vouchers, visit vouchers, and other types of entry permits officially issued by FITNESS PLUS MANAGEMENT.
2. FITNESS PLUS GUESTS can use FITNESS PLUS facilities in the following procedures:
   a. Fill out the guest book and sign the disclaimer agreement listed in the guest book.
   b. Submit an official ID in place of a MEMBER card during practice.
   c. For trial voucher users, they are eligible to use this facility.
   d. Users of 1-day training vouchers can enjoy the facility after completing various payment procedures in accordance with the provisions.
   e. Employees or services/nannies of FITNESS PLUS MEMBERS and/ or GUESTS are not permitted to enter the FITNESS PLUS training area.
   f. Children under the age of 14 are not allowed to enter the FITNESS PLUS training area.
   g. FITNESS PLUS GUESTS are required to fill out and sign the list of available guests.

RESPONSIBILITY

1. FITNESS PLUS MEMBERS and/or FITNESS PLUS GUESTS are prohibited from bringing or placing personal belongings into the gym area or lockers. FITNESS PLUS MANAGEMENT shall not be held responsible for such personal belongings, including but not limited to any negligence, loss, theft or damage.
2. FITNESS PLUS MEMBERS and/or FITNESS PLUS GUESTS are obligated to acknowledge and be able to consciously measure their health condition (must be in prime condition) before using the FITNESS PLUS tools, equipment and/or facilities. FITNESS PLUS MANAGEMENT is not responsible for any consequences that may be caused by such negligence, including but not limited to those resulting in illness, injury, and even death.
3. Users of FITNESS PLUS facilities must have a medical examination before joining FITNESS PLUS and/or declare themselves sick.
4. FITNESS PLUS MEMBERS are required to check in before entering FITNESS PLUS.
5. FITNESS PLUS GUESTS are allowed to use the facility by giving a one-day workout voucher according to the price at the time of visit.
6. FITNESS PLUS MEMBERS AND/OR GUESTS are not permitted to conduct all trade transactions on FITNESS PLUS without obtaining approval from FITNESS PLUS MANAGEMENT.
7. Any issues, or disputes that occur in connection with FITNESS PLUS will be resolved on a familial basis.
8. The failure of a FITNESS PLUS MEMBER and/or GUEST to regularly train and use part or all of the FITNESS PLUS facilities does not waive the FITNESS PLUS MEMBER and/or GUEST to pay the fees specified by FITNESS PLUS MANAGEMENT as indicated in the rules and conditions, regardless of the reason.
9. FITNESS PLUS MANAGEMENT has the authority to supplement the existing regulations applicable to FITNESS PLUS in accordance with the conditions and needs of FITNESS PLUS MANAGEMENT (if required) without the obligation to give prior consent and obtain the prior consent of the FITNESS Plus MEMBER and/or GUEST and all such rectifications shall be binding on the FITNESS PLUS MEMBER and/or GUEST to be complied with upon notification from FITNESS PLUS MANAGEMENT.

DISCLAIMER

1. Each FITNESS PLUS MEMBER must read, understand, and sign the FITNESS PLUS MEMBERSHIP Code of Conduct & Rules Form which is an integral attachment to the MEMBERSHIP CONTRACT.
2. The original signed Disclaimer Form is stored in the FITNESS PLUS MEMBERSHIP database.
